For those students who have their own comparable insurance program and request to waive out of the program, please download the Alternate Insurance Waiver Form and complete with your personal information. You must then send the Alternate Insurance Waiver Form directly to your insurance company and have them complete . Once your insurance company has completed their section(s), the insurance company must send the completed form directly to Insurance For Students, Inc.
